Reports and private medical request

Use this service to request a medical report.

You can use this service if you:

  • are registered at the surgery

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

There is a charge for this service between £16 and £150. You must bring any necessary forms to reception and we will process your request and book any necessary appointment for you (if required). A deposit will be taken for Taxi medicals and HGV driver medicals.
